🔧 Medium stake tools with per-agent approval history

Introduced a new medium stake level for tools that maintains approval history per agent rather than globally
Medium stake is now the default for Gmail drafting tools, Slack message posting, and Google Calendar event editing
Admins can configure the medium stake level for any tool
Users will be prompted to approve medium stake tools on a per-agent basis
